DEATH RECORDS ARE CONFIDENTIAL AND ONLY AVAILABLE TO QUALIFIED APPLICANTS

DEATH RECORDS ARE NOT PUBLIC RECORD UNTIL 25 YEARS AFTER THE DATE OF DEATH 

 

Applicants

Death Certificates are only issued to Walk-In Applicants or Mail-In Applicants. We cannot process phone-in or faxed requests. 

Requirements for Walk-In Applicants:

  • A completed Application for Birth or Death Record
  • An original, valid government-issued photo ID
  • Payment in Full

Requirements for Mail-In Applicants:

  • A completed, State approved, Application for Birth or Death Record

The Application must include the following information:

  • Full name as it appears on death record
  • Date of death
  • Your relationship to person named on certificate
  • Your valid mailing address
  • Purpose of record
  • Signature and date
  • A daytime phone number
  • A completed, notarized Proof of Identification & Affidavit (Pg. 2 of the Application)
  • A legible copy of a valid government-issued photo ID
  • Payment in Full (DO NOT MAIL CASH)

 Applications are available in our Office or they can be download from the links at bottom of this section.

Qualified Applicants

Qualified Applicants Include:         

  • Registrant or an Immediate Family Member by blood, marriage or adoption;
  • Guardian or Legal Representative;
  • Law Enforcement or Governmental Agencies;
  • Persons who can demonstrate a direct and tangible interest in the record for statutory obligations; or
  • Persons protecting a personal legal property right and who can present an Original Designation Document or Attested Statement.

Valid Government-Issued Photo ID is Required

 

Fee Schedules - Effective January 1, 2024

ALL fees are due and payable at the time of filing. Personal checks are not accepted. Attorney's business checks are accepted. Visa and Master Cards accepted. Fees may be charged to a credit card via telephone or mail by supplying the account number, expiration date, and card holder's zip code.
